Choice Advocates of Tampa Bay is dedicated to providing high quality community based services, support , and training. C.A. is willing to increase awareness of what our community has to offer , provide postive social interactions , help develop natural support, increase independency and establish collaborative effort to work together with circle of support to obatin and maintain their personal goals! Choice Advocates of Tampa Bay will maintain a 24-hr on call system in the event of an emergency. C.A. has experience staff on board and provides services around the Tampa Bay area to all persons Developmental Disabilities, Blind, Deaf and Hard of Hearing that meet qualifications and is eligible through Developmental Services Home and Community Based Waiver program and/or private/independent care. .In-home Support... Overview: In Home Support provides between four and twenty-four hours per day of assistance and/or supervision in the clients' home to ensure their saftey.The staff may live in the individuals home or apartment and share living expenses with the individual.The support staff will provide companionship and/or personal care, and may assist with actvities of daily living and those actvities which enable the individual to reside in their own home.For monitoring review purposes and to meet all criteria for providing Supported Living Services; each case file will consist of all documents pertaining to that individual and their specfic services for example; Implementaion Plan; Support Plan;Medical Records etc. Companion Services...
Overview: Companion services are designed to provide training in specfic areas of daily living,home maintenance,cooking and social and safety skills which are necessary to prepare a person to live more independently in the community. The Support Staff will provide training and guidance to ensure the acquisition,retention or improvement of skills needed to live independently. This service is provided to reflect the individual's needs as definded in their Individual Support Plan, Functional Assessment, and Implementaion Plan to inculde each customer's level of behavioral and functional needs. These services inculde, but are not limited to, actvities of daily living such as personal hygiene, household chores, meal planning and prepartion, shopping, personal fitness, and social and adaptive skills. | Supported Living Services......
Supported living is an opportunity for adults with developmental disabilities to choose where, how and with whom they live. People receive personalized supports needed to maintain their own private home. Through supported living, individuals, even those with severe disabilities, can receive support to live in their own homes. Supported Living is designed to provide support and training necessary to provide an individual the opportunity to live independently in the community. Supported Living Coaches will provide training to ensure aquistion, retention, or improvement of skills needed to learn independently.The supported living service will reflect an individual's needs and may be incorporated in daily actvities, personal hygiene, household chores,menu planning, meal preparation, shopping, personal finances, and social and adaptive skills. |
Supported Employment... Supported Employment is designed to support and assist individuals with obtaining job skills and job placement within their community.Supported Employment Coaches will assist individuals in completing an implementaion plan according to their support plan and chosen goals.Services include but are not limited to; assessing job skills, assisting in job searches, reviewing employment advertisements, completing job applications, developing a resume, and developing interview skills. |
